HAIR LOSS HEALING COLLECTIVE
CLIENT COACHING AGREEMENT
This Client Coaching Agreement (“Agreement”) is entered into between Kat Spangler, operating as Follow Your Gut Nutrition (hereafter referred to as “Coach”), and the individual purchasing the Hair Loss Healing Collective (hereafter referred to as “Client”).
The purpose of this Agreement is to establish the terms, responsibilities, expectations, and limitations associated with participation in the Hair Loss Healing Collective.
By completing enrollment and accepting this Agreement, Client agrees to the following terms.
1. PROGRAM SERVICES
The Hair Loss Healing Collective is a three-month, personalized nutrition and lifestyle coaching program designed to help Client investigate potential contributing factors to hair loss and support overall health and hair wellness.
Services include:
-
Review of Client's completed health history, symptom questionnaires, and available laboratory results.
-
Individualized recommendations for functional laboratory testing, when appropriate.
-
Educational interpretation of laboratory findings from a functional nutrition perspective.
-
A personalized 90-day nutrition, supplement, and lifestyle protocol, delivered in three 30-day phases.
-
Monthly progress check-ins and adjustments to recommendations based on Client's responses.
-
Direct messaging support through Slack during the designated coaching period.
Private consultations, telephone or video calls, and weekly office hours are not included unless purchased separately or expressly agreed to in writing.
Laboratory testing, supplements, medical appointments, and other third-party products or services are purchased separately and are not included in the program price.
2. ONBOARDING AND PROGRAM TIMELINE
Client agrees to complete and submit all required intake forms, available previous laboratory results, and any recommended laboratory testing within 45 days of enrollment.
Timely completion of onboarding requirements is necessary for Coach to develop an individualized protocol.
The 90-day coaching period ordinarily begins when Coach delivers Client's initial personalized protocol.
However, delays caused by Client's failure to complete onboarding requirements within 45 days may reduce the remaining coaching period and will not automatically extend access to services.
Delays attributable to laboratory processing times will be accommodated as reasonably determined by Coach.
Client understands that enrollment, payment dates, onboarding, and the coaching period are separate processes and may not begin or end on the same dates.
3. MONTHLY CHECK-INS AND PROTOCOL ADJUSTMENTS
Client is responsible for completing designated monthly check-in forms according to the instructions provided.
Coach will use Client's responses to evaluate progress and make appropriate adjustments to the next 30-day protocol.
Submission of each required check-in is necessary before the next protocol update can be prepared.
Late, incomplete, or missed check-ins may delay protocol adjustments but will not extend the established coaching period.
Client acknowledges that consistent participation and communication are important to receiving the intended benefit of the program.
4. MESSAGING SUPPORT AND COMMUNICATION
Client will receive access to a designated Slack communication channel.
During onboarding, before delivery of the initial protocol, messaging support is limited to questions concerning intake forms, laboratory testing, and onboarding procedures.
Personalized coaching and protocol-related messaging begin upon delivery of the initial protocol.
Coach's standard communication hours are:
Monday–Thursday, 9:00 AM–6:00 PM Central Time.
Coach aims to respond to messages within 48 business hours, excluding weekends, holidays, and scheduled closures.
Messaging support is intended for questions, clarification, and troubleshooting related to the established protocol. It does not include emergency assistance, continuous monitoring, or unlimited on-demand consultations.
Messaging support ends when Client's 90-day coaching period concludes, subject to the onboarding deadline provisions in Section 2.
5. PAYMENT TERMS
Client agrees to a total program investment of $750, paid in three automatic monthly installments of $250.
The first payment is charged upon enrollment, and the remaining two payments are automatically charged on the corresponding monthly billing dates.
Client authorizes these recurring charges as part of the agreed three-month payment commitment.
All three payments are required regardless of onboarding delays, incomplete forms, missed check-ins, or Client's level of participation.
The payment schedule begins upon enrollment and is not postponed or adjusted based on the date the initial protocol is delivered.
Laboratory testing, supplements, medical appointments, and additional services are separate expenses.
If a payment fails, Coach may suspend services until the outstanding payment is resolved, following reasonable notice to Client.
Suspension of services due to nonpayment does not automatically extend the coaching period or eliminate the remaining payment obligation, subject to applicable law.
6. CANCELLATION AND REFUND POLICY
Enrollment constitutes a three-month financial commitment.
Client understands that the program includes individualized preparation, review, recommendations, and reserved coaching availability.
Except where otherwise required by applicable law or expressly agreed to in writing, payments are nonrefundable, and discontinuing participation does not eliminate the remaining payment obligation.
Any request for an exception must be submitted in writing and is subject to Coach's review and applicable law.
7. SCOPE OF PRACTICE
Client acknowledges that Coach is a functional nutrition practitioner and personal trainer and is not acting as a physician, dermatologist, psychologist, or other licensed medical provider.
The Hair Loss Healing Collective provides individualized nutrition and lifestyle education and coaching. It does not provide medical diagnosis, medical treatment, prescriptions, or medical management of disease.
Client understands that hair loss may result from a variety of factors, including genetic, hormonal, nutritional, inflammatory, autoimmune, medication-related, or other medical conditions.
Certain types of hair loss, including scarring alopecias such as lichen planopilaris, may require timely evaluation and ongoing treatment by a qualified dermatologist.
Participation in this program is not a substitute for dermatological evaluation, prescribed treatment, or other appropriate medical care.
Coach does not advise Client to discontinue, delay, or replace medically prescribed treatments.
Client agrees to consult an appropriate licensed healthcare professional regarding medical symptoms, diagnoses, laboratory abnormalities, medications, and the safety of supplements or dietary changes when appropriate.
8. LABORATORY TESTING AND INTERPRETATION
Coach may recommend laboratory testing to help Client explore nutritional status, metabolic function, or other potential contributors to overall health.
Client understands that:
-
Laboratory testing may be optional or recommended based on individual circumstances.
-
Testing fees are separate from program fees.
-
Laboratory results may require medical interpretation or follow-up with a licensed healthcare provider.
-
Functional nutrition interpretations are educational and are not medical diagnoses.
-
Laboratory findings do not necessarily establish the cause of hair loss.
-
Coach cannot guarantee that testing will identify a specific underlying cause.
Coach may recommend that Client seek additional medical evaluation when symptoms or laboratory findings warrant it.
9. CLIENT RESPONSIBILITIES
Client acknowledges responsibility for:
-
Providing accurate and complete health information.
-
Completing intake forms and check-ins in a timely manner.
-
Communicating relevant changes in health, symptoms, medications, or treatment.
-
Reviewing recommendations and seeking medical clearance when appropriate.
-
Making independent decisions regarding dietary, lifestyle, and supplement changes.
-
Participating consistently and communicating when recommendations are difficult to implement.
Client understands that individual results depend on numerous factors, some of which are outside Coach's control.

11. WAIVER OF LIABILITY AND ASSUMPTION OF RISK
Client acknowledges that participation in the Hair Loss Healing Collective may involve changes to nutrition, dietary supplements, physical activity, and lifestyle habits.
Client understands that these changes may involve risks, including but not limited to allergic reactions, digestive symptoms, supplement interactions, changes in medication requirements, or other adverse effects.
Client agrees to disclose relevant medical conditions, medications, allergies, and other health information that may affect the safety or suitability of recommendations.
Client voluntarily assumes the ordinary risks associated with independently implementing educational nutrition and lifestyle recommendations and agrees to consult an appropriate licensed healthcare professional when medically indicated.
